A Clinical and Care Team that Works for You.
Prior to the start of services, a Samaritan Health Service nurse will first visit your home to evaluate the needs of you and your loved one. We will then work with your physician to develop a customized care plan and determine the level of care required. Based on the needs and the care prescribed, the plan may include visits from:

Nurses, for medical needs

Clinicians for skilled care

Certified Nursing Assistants (CNAs) to provide personal care assistance when applicable
Your Samaritan Health nurses, clinicians, and caregivers are all trained and certified to deliver safe, quality care and they work together to provide the best outcomes and service experience possible.

Payment Options
After your in-home assessment and a care plan is created, we can provide you with the hourly rates for our services. We will help you find out if the services you need are covered by insurance or another benefit such as Medicare, Medicaid, the VA. We are here to help you, every step of the way.
